rom the Departments of Internal Medicine III, Vascular Surgery, and Paediatric Cardiology, niversity of Cologne, ologne, Germany. anuscript received ay 4, 2009; ccepted May 12, 2009. A46-year-old man had poorly controlled arterial hypertension despite triple antihypertensive medication for years before by chance aortic coarctation was diagnosed. He was admitted to our institution for operation. Given the high operative risk due to the marked collaterals, we decided on an attempt at percutaneous dilation of the coarctation in surgical stand-by. However, via the femoral artery, the ascending aorta could not be reached. Angiography in the ascending aorta via the right brachial artery (A, Online Videos 1 and 2) and descending aorta (B) confirmed the diagnosis of aortic atresia with blind-ended proximal and distal aorta separated by 3 mm and marked collaterals. As an attempt of recanalization from the brachial approach was unsuccessful, interruption of the aortic arch was recanalized and reconstructed with a Brockenbrough needle (C, Online Videos 3 and 4) and subsequent implantation of a covered stent (CheathamPlatinum stent, NuMED Inc., Cornwall, Ontario, Canada) (D, Online Videos 5 to 8) from the femoral side without any complications. During follow-up, blood pressure could be controlled with standard doses of an angiotensin receptor blocker and beta-blocker underscoring the need to exublished by Elsevier Inc. doi:10.1016/j.jacc.2009.05.079

Customized covered stent graft for percutaneous closure of Fontan baffle leak.
We report the innovation of both a partly-covered and completely-covered, variable-diameter, balloon-expandable stent that was custom-designed by NuMed, Inc for percutaneous closure of a baffle leak after total caval pulmonary connection (TCPC). A 50-year-old patient, born with tricuspid atresia, who had undergone TCPC, developed severe persistent cyanosis due to a right-to-left shunt through a...
متن کاملHemodynamic Performance of a Novel Right Ventricular Assist Device (PERKAT)
Acute right ventricular failure (RVF) is an increasing clinical problem and a life-threatening condition. Right ventricular assist devices represent a reasonable treatment option for patients with refractory RVF. We here present a novel percutaneously implantable device for right ventricular support. The PERKAT device is based on a nitinol stent cage, which is covered with valve-carrying foils....
متن کاملClinical evaluation of covered stents in the treatment of superficial femoral artery pseudoaneurysm in drug abusers
The present study aimed to investigate the technical feasibility and initial clinical outcomes of a covered stent for the endovascular treatment of superficial femoral artery (SFA) pseudoaneurysm in drug abusers. A total of 29 drug abuse patients with SFA pseudoaneurysm, as confirmed by color Doppler sonography, were enrolled to the present study between January 2012 and May 2014. All patients ...
متن کاملCovered endovascular reconstruction of aortic bifurcation (CERAB) technique: a new approach in treating extensive aortoiliac occlusive disease.
Endovascular treatment of occlusive disease of the aortic bifurcation is challenging. We developed the Covered Endovascular Reconstruction of Aortic Bifurcation or CERAB-technique, as a new approach for extensive and/or recurrent aortoiliac occlusive disease using three covered balloon expandable stents to reconstruct the aortic bifurcation. This configuration provides the ability to deal with ...
متن کاملTranscatheter stenting of arterial duct in duct-dependent congenital heart disease.
INTRODUCTION Critical congenital heart diseases (CHD) are mostly duct-dependent and require stable systemic-pulmonary communication. In order to maintain patency of the ductus arteriosus (DA), the first line treatment is Prostaglandin E1 and the second step is the surgical creation of aortic-pulmonary shunt. To reduce surgical risk in neonates with the critical CHD, transcatheter stenting of DA...
